Health Forms

Medical forms must be completed and submitted on or before June 1, 2020. The State of Connecticut and local Board of Health regulations require complete health records for all participating students and staff. These records include the following:

• Connecticut Health Assessment Form
• Authorization to Administer Medication Form
• Food Allergy Emergency Care Plan Form
